PARENTS OF CURRENT OR PROSPECTIVE ATHLETES

If your child plans to participate in sports in the fall, they must supply the health office with a physical dated after August 2010.  If they do not have a physical after that date, please schedule one with your health provider as soon as possible.   Have your child bring a copy of their physical along with their completed & signed sports verification form (tri-fold form) on the scheduled sports verification date below. Sports verification forms will be mailed to athletes over the summer or can be obtained in the main office or athletic office. The tentative dates are as follows:

Williamsville East:

Monday, August 22

12:30-Girls & Boys Golf

ONLY A PARENT CAN SUBSTITUTE FOR AN ABSENT ATHLETE (not friends)  

These dates are subject to change.  Please check with your coach or the athletic dept. to confirm dates for verification.

Your child will not be allowed to participate unless they bring the trifold form to their first practice.
